New Delhi: The Ministry of Health and Family Welfare (MoHFW), Government of India, has implemented a Self-Reporting Form for international passengers arriving in the country. The measure is aimed at strengthening health monitoring and ensuring the safety of travelers and residents.

Under this requirement, passengers must provide important details such as their passport information, flight number, nationality, age, arrival date, and contact address in India. Travelers are also required to share their travel history for the previous 28 days and disclose any health symptoms, including fever, cough, or respiratory distress.
The form includes a declaration section where passengers confirm the accuracy of the information provided. Authorities have stated that false information or non-cooperation may lead to legal action under applicable regulations.
Health officials say the form plays a key role in identifying potential health risks and enables authorities to respond quickly when necessary. Travelers who develop symptoms after arrival are advised to contact health authorities and follow medical guidance.
Passengers are encouraged to complete the form carefully before reaching the Health and Immigration Counter to ensure a smooth arrival process. The initiative reflects India’s continued efforts to maintain effective public health surveillance at international entry points.
